A Clinically Focused Team
Through their tireless work, prodigious talent and trust, our team at Spring Hills is committed to implementing new ideas to improve people’s lives and make the health care industry smarter, kinder, and better overall. Combining 20 years of post-acute and long-term care experience with the expertise of clinicians, specialists and health system veterans Spring Hills enhances the recovery process from a hospital stay and ultimately reduces post-discharge complications and readmissions.
With the goal of reducing hospital readmissions, our team has developed Population Health, a clinical healthcare model to support people with chronic conditions at every stage of their journey. The journey begins when someone is admitted to a Spring Hills center where a clinical and social work assessment is performed to evaluate how to best serve patients. Gloria Berwick, The Vice President of Operations for Population Health described the program to be similar to that of GPS navigation; “Before you embark on your journey, route options with different lengths of time, traffic factors and tolls. With Population Health, as the navigator, we help you decide which is the best healthcare route to take.”

This is why we firmly believe in having a team comprised of experienced, knowledgeable and passionate individuals that believe in our vision for the future of healthcare. It is a privilege to have recently added Pierantonio Russo, MD to our team.
Pierantonio Russo, MD – Chief Medical Officer and Chief Clinical Transformation Officer of the Population Health Management Program.
Russo will oversee the integration of Population Health into Spring Hills’ clinical programs and offer guidance to providers, payers, and policymakers on IT and AI enabled comprehensive population management, including complex care for high-risk, high cost, high needs patients. “Dr. Russo is a clinician with a notable track record of driving new innovation to improve patient care and outcomes,” said Alex Markowits, Founder and President/CEO of Spring Hills. With his experience in cardiology, Dr. Russo also will be a key asset to our specialized cardiac program for people transitioning from hospital to home after a cardiac event.”

With the vision of expanding to include clinical programs that will offer residents and patients uniquely comprehensive health care, the establishment of the Cardiac Program with integrated Population Health Management, is just the beginning.
